Benefits include a $10 copay for an annual WellVision eye exam; a $20 copay for Primary Eyecare visits; a $25 copay for single, bifocal, or trifocal lenses; allowances for frames and contact lenses; and discounts on LASIK surgery. The plan provides these benefits every 12 months.
